Sacroiliac Joint Dysfunction Physiotherapy: Symptoms & Treatment

Sacroiliac Joint Dysfunction is a condition that causes pain and discomfort in the lower back, buttocks, and legs. Physiotherapy can help by providing exercises and manual therapy to improve joint mobility and reduce pain.

The sacroiliac joint (SIJ) is the joint between the sacrum and the ilium, or your tailbone and pelvic bone. You have an SI joint on either side of the lowest part of your back. This joint helps with shock absorption between the upper and lower body.

Sacroiliac joint dysfunction can be painful and interfere with your active lifestyle. Pain that resides at the dimple of the lower back and travels into the gluts, upper thigh and groin may be an indication of SIJ dysfunction. You may also have difficulty standing up after sitting, using stairs, or rolling over in bed.

Three types of SIJ dysfunction are:

  • Hypermobile – moves too much and is unstable
  • Hypomobile – stiff or shifts out of position and gets stuck
  • Sacroiliitis – inflammation of the joint

Common causes of SIJ Dysfunction are:

  • Trauma from a fall
  • Postural errors that consistently place more weight through one side of your pelvis such as: crossing your legs, leaning on one arm rest or tucking legs under you while you sit, sitting on a wallet regularly or carrying a child on one hip
  • Various types of arthritis cause inflammation and pain
  • Pregnancy or Post-Partum Hormones: when the hormones present during this time, they relax the ligaments and the joint could shift and lock out of place
  • One leg longer than the other increases the pressure on one side that travels up the knee and hip joints
  • Muscle imbalances or weaknesses around the hips/gluts/low back that cause even slightly poor mechanics, which when repeated with running, walking or exercise, can cause pain

Your pelvic floor physiotherapist can assess the cause of your pain and treat the joints and the surrounding muscles in the hip and lower back to relieve the pain and discomfort while correcting any biomechanical imbalances. They will use specialized techniques to restore the normal alignment and mobility of the SI joints. They can also provide exercises that will help stabilize and support the area as well as stretch out the tense muscles.

Registered Massage Therapy can also be of great value with SI joint dysfunction. There is generally a lot of muscle tension and spasm around the joints through the hips and low back that can be relieved with Massage Therapy to help lessen pain and allow the joints to move and align better. Massage Therapy paired with Physiotherapy or Athletic Therapy can be a winning combination for this injury.
